MEDICAL EVAL QUESTIONNAIRE (MEQ) MUST BE COMPLETED BEFORE WORKPLACE RESPIRATORS ARE FIT TESTED.(See "About" tab for more info)

• Bioenvironmental Engineering conducts fit tests on Tuesdays from 0800-1100, 1300-1500. Please BRING YOUR MASK with you, or you will be turned away. You must be clean-shaven and please do not eat, drink, smoke or chew tobacco 15 minutes prior to your appointment.
• For Workplace specific respirators you must have a FAFB Medical Eval Questionnaire on file. (Contact Public Health at 247-5757 if you need to complete one.) Workplace specific respirators are provided by the workplace.
• Gas Masks can be picked up from IPE (Bldg 2163)(DSN 247-4930). Their hours are Mon-Wed/Fri: 0700-1400. Unless you are deploying, member needs to provide proof of a scheduled fit test to IPE to receive the mask.
•If you have already completed a gas mask fit test and need a copy of the report, please call us at 247-2391.
